Preliminary findings from an internal investigation by the Nigerian Railway Corporation (NRC) have pointed to the possible sudden development of a bogie or wheel defect as a potential primary factor in the derailment involving the Warri–Itakpe Train Service (WITS) on June 8, 2026.

According to the findings, the suspected defect may have occurred while the train was en route and could have resulted in abnormal wheel-rail interaction, excessive impact loading and a loss of running stability.

The NRC inquiry team also identified the possible manner in which the brakes were applied as a factor that may have contributed to the severity of the incident.

However, the corporation stressed that the observations remain working hypotheses, noting that the definitive cause or causes of the derailment would only be established after a comprehensive analysis and documented in the final investigation report.

The inquiry team further disclosed that its examination of the incident site found the railway points intact, with no evidence of track vandalism.

This finding differs from the WITS accidents recorded on November 1 and November 8, 2025, which were attributed to track vandalism.

Meanwhile, in line with statutory requirements, the Nigerian Safety Investigation Bureau (NSIB) has commenced an independent investigation into the June 8 incident.

The Nigerian Railway Corporation said it is fully cooperating with the NSIB investigation and will be guided by the bureau’s findings and recommendations.

The NSIB’s final report on the incident is still pending.
